Can a 1,700-year-old medical text lead to the eradication of one of the world’s deadliest diseases? It sounds unlikely, yet that is how scientist Tu Youyou discovered the cure for malaria in the middle of a cultural revolution in China – a breakthrough that transformed malarial treatment and redefined the relationship between traditional knowledge and modern science.
Malaria, as we know today, is a widespread and potentially lethal human infectious disease. It has been the reason for increasing mortality rates all over the world for many centuries. For thousands of years, scientists have worked on different Plasmodium models to find an effective cure to this infectious disease. During times of war, people have suffered to death due to the unavailability of an effective cure. After screening many random chemicals, scientists turned to traditional herbal remedies.
By the mid-1960s, malaria had become one of the world’s most devastating infectious diseases. The crisis was particularly severe during the Vietnam War. The soldiers and their allies suffered heavy losses due to the epidemic. On some battlefields, the epidemic was responsible for reducing military strength by half and, in severe cases, disabling 90% of the troops. In 1966, North Vietnam sought assistance from China in finding a more effective treatment for this disease. Recognising both humanitarian and strategic significance, Chairman Mao Zedong approved a secret nationwide research initiative on 23rd May 1967. The project was codenamed Project 523. Rather than being led by a single scientist, the mission brought together thousands of scientists from dozens of institutions in China. Among them was pharmacologist Tu Youyou, who led a team investigating traditional Chinese herbal medicine and eventually discovered an effective cure for malaria.
The year before Project 523 was launched, Chairman Mao Zedong had introduced the Cultural Revolution to renew the spirit of the Chinese Revolution. It was a socio-political movement to strengthen communist ideology and to mobilise the urban youth of the nation. The movement quickly spread across the country, which eventually resulted in the shutdown of schools and universities. Scientific research and higher education were severely disrupted, bringing many academic projects to a standstill. Despite this turmoil, some government-backed strategic research programs continued under strict secrecy. One such initiative was Project 523. It was within this challenging political environment that Tu Youyou and her colleagues carried out their research.
When Project 523 was launched, China needed scientists who could think beyond conventional medicine. Among those selected was Tu Youyou, a young aspiring pharmacologist whose unique background and insights in both modern pharmaceutical science and traditional Chinese medicine made her an ideal candidate for the mission.
During her teenage years, Tu Youyou suffered from a tuberculosis infection that interrupted her high-school education. As she spent most of her days suffering from tuberculosis in isolation, she became curious about acquiring knowledge about different medical practices. This experience sparked her interest in medicine and inspired her to pursue a career dedicated to treating diseases. She was later admitted to the Beijing Medical College, where she studied pharmaceutical science. As technology was advancing day by day, scientist shifted their practices from traditional to modern pharmacology. Unlike other researchers of her time, Tu Youyou was trained in both modern pharmacology and traditional Chinese medicine. She learned how to analyse medicinal herbs using scientific methods while also studying ancient Chinese medical texts and herbal therapies. This rare combination became the foundation of her greatest discovery.
Project 523 was a mission that involved 500+ scientists across 60 prestigious medical institutions in China. Therefore, the mission was led by a team of highly knowledgeable scientists who were experts in their respective fields. Inspired by the new scientific innovations, most of the scientists experimented through various modern pharmacological techniques that mainly involved various synthetic compounds.
In 1969, Tu Youyou was then 39 years old and decided to investigate Chinese herbs. She visited practitioners of traditional Chinese medicine all over the country on her own. She combined ancient medical knowledge with modern pharmacology to identify, isolate and validate a new antimalarial drug. As part of the project, Tu Youyou and her team searched for clues in traditional Chinese medicine. They systematically reviewed more than 2,000 ancient herbal recipes. They studied folk remedies used to treat fever from hundreds of classical medical books. As fever is a key symptom of malaria, they thought of treating the patients using folk remedies. They studied, experimented and compiled over 640 herbal prescriptions that might have antimalarial potential. During Tu Youyou’s search for traditional remedies, she came across one particular text, “The Handbook of Prescriptions for Emergencies”, written around 340 CE by the physician Ge Hong. The text described the procedures to treat intermittent fevers using sweet wormwood (Artemisia annua). Using low-temperature extraction methods, Tu Youyou and her team were able to extract the active compound of Artemisia annua. The extracted compound was later termed ‘Artemisinin’. Laboratory studies and clinical trials showed that artemisinin rapidly killed the malarial parasite, even in cases where older drugs had become ineffective.
After successful extraction of Artemisinin, it was important to test this drug to ensure its effectiveness. Animal testing was successful, but a necessity arose among the scientists to determine the effectiveness in the human body. Although drug testing on humans would be considered unethical today, despite the complications and the risks, Tu Youyou, along with her colleagues, voluntarily tested the drug on themselves. Following these preliminary tests, the drug was administered to the patients with malaria.
The Nobel Committee recognised Tu Youyou as a pioneering scientist who led the research of Artemisinin. They viewed her innovative discovery as an extraordinary contribution to humanity. Despite not having any formal medical degree or a PhD, Tu Youyou was awarded the Nobel Prize in 2015 and gained worldwide recognition for her groundbreaking discovery.
The discovery of Artemisinin is celebrated as one of the greatest medical breakthroughs of the twentieth century. However, it has also been the subject of debate regarding how credit should be distributed. Supporters of Tu Youyou praised her unique insights that led to the discovery of the cure. Others believed that the discovery was successful due to a much greater collaborative effort.
Despite all the socio-economic, ethical and political debates, the discovery of Artemisinin was remarkable as it incorporated traditional as well as modern practices. Thanks to the contribution of Tu Youyou, her team, and all the other scientists working in Project 523, a new and effective cure for malaria has been discovered.
At present, Tu Youyou’s legacy continues to inspire young scientists around the world. Her story is now regarded as one of the greatest examples of testing traditional knowledge through modern practices that benefit humanity worldwide.
